Zusammenfassung: Falldarstellungen als Instrument diagnostischer und therapeutischer Qualitätssicherung enthalten neben der Beschreibung der Diagnostik und der ICD-Diagnosen ein modellbasiertes Fallkonzept, aus dem Therapieziele abgeleitet werden, deren Umsetzung und Evaluation dokumentiert werden. Besondere Bedeutung haben Falldarstellungen in der Aus- und Weiterbildung. Standardvorlagen der Klinischen Neuropsychologie sind historisch bedingt an Befunden und Gutachten orientiert. Entwicklungsbedarf wurde bereits seit Inkrafttreten der alten Weiterbildungsordnung der Landeskammern vor 10 Jahren und des aktuellen Curriculums der Gesellschaft für Neuropsychologie 2017 deutlich. Dies gilt umso mehr, seit die neue Weiterbildungsordnung in Kraft getreten ist, die die neuropsychologische Psychotherapie den anderen Psychotherapierichtungen gleichstellt. Besonderer Bedarf besteht an Standards für die Entwicklung individueller Fallkonzepte als Grundlage für Therapieziele und therapeutisches Vorgehen. Im Sinne dieser Zielsetzung skizzieren wir hier eine standardisierte Falldarstellung mit ICF-basierter Fallkonzeptualisierung, die neben funktionellen Defiziten auch Aktivitäts- und Partizipationseinschränkungen, Person- und Umweltfaktoren sowie eine fachlich begründete, abgestufte Auswahl von Therapiezielen enthält. Sie soll der Fachöffentlichkeit als Diskussionsgrundlage für die Weiterentwicklung neuropsychologischer Standards dienen.

As an instrument of diagnostic and therapeutic quality assurance, case reports contain a description of the diagnostic procedure and the resulting ICD diagnoses and a model-based case concept. Therapy goals are derived and the implementation and evaluation of these goals are documented based on this concept. Case reports are of particular importance in training and further education. Historically, standard tem-plates in clinical neuropsychology have been based on findings and expert reports. Candidates and supervisors have formulated the need for development since the previous state-decreed training regulations came into force 10 years ago, and in 2017, the current curriculum of the German Society for Neuropsychology was introduced. The new further training regulations put neuropsychological psychotherapy on an equal footing with the other fields of psychotherapy. There is a particular need for standards for developing individualized case concepts as a basis for therapy goals and therapeutic procedures. In line with this objective, we outline here a standardized case presentation with ICF-based case conceptualization, which, in addition to functional deficits, also includes the domains of activity and participation. We include personal and environmental factors as well as a professionally justified, graduated selection of therapy goals. This outline serves the professional commu-nity as a basis for discussion for the further development of neuropsychological standards.
